For the sake of this discussion, I'm not including PMMA because it is considered a non surgical method, even though PMMA is permanent and can be compared to the similar methods of application of FFT (free fat transfer), FFT is only considered a surgical procedure because of the harvesting methods of the fat to be injected. There is some buzz about FFT and stem cells, but from what I can tell, the whole - stem cell FFT - information seems to be just old phalloplasty clinics repackaging an old product so it sounds like some modern breakthrough. Yes, I bet all FFT and blood has some degree of stem cells in them, but I don't think the stem cells they're talking about are in fact - penis tissue stem cells - for new penis growth.

For this discussion, I've included FFT, but not PMMA, or engineered penile erect tissue procedures. Free Fat Transfer is the harvesting of adipose tissue via liposuction and injecting it into the penis with a cannula. Initial results are similar to PMMA, but FFT longevity is questionable. From hundreds of personal accounts from men, FFT is often reabsorbed by the body and can leave an unsightly lumpy appearance from uneven absorption. The most up to date plastic surgeons also share this opinion of fact, Fat Injection to increase penis girth. With the longevity of FFT results in question, FFT is probably one of the safest methods in the application of surgical penis enlargement for girth. Penis Grafting Surgeries
Belladerm, Alloderm, and Dermal grafting for penis girth is probably the most intensive method of phalloplasty. Belladerm and Alloderm are donor tissues that can add a significant amount of penis girth. Belladerm is harvested from living donors, and Alloderm is harvested from cadaver donors. All genetic and DNA material is stripped from the tissue leaving a blank slate.
The acellular dermal matrix grafts are surgically implanted under the skin and attached to the tunica. Belladerm and Alloderm have also been used to enlarge the glans penis. In recent years, Belladerm has become the more popular tissue utilized for penis enlargement surgery and is considered a permanent enlargement. There have been some reports of the longevity of Alloderm tissue implanted in the penis being less than permanent, shrinking down and hardening several years after implantation.
Dermal grafts are tissue grafts harvested from your own body. Also referred to as, autologous dermal grafting, the harvested tissue can add several inches of erect circumference and is considered permanent. Similar to Belladerm and Allografts, dermal grafting is a very intensive and risky surgery. Any surgery where there is tissue grafting and, or, tissue implantation there are many risks involved. My dermal grafts were harvested from my lower back, and there was additional risk involved from the harvesting area.
Over the years I've been in contact with many men that have had dermal grafting and donor grafts, in the earlier days there were a lot more complications associated with this type of surgery. Grafting techniques and surgical methods have come a long way since my procedure, but I don't think I will opt for additional grafts in the future because of how intense the whole process is.
The healing time for grafting enlargement surgeries can be several months. Currently, grafting methods have improved by techniques of insertion and limiting incision sites. Multiple incision sites also mean multiple points of entry for infections to pass into that area. Limiting the need for multiple incisions to insert the grafts material has improved both the safety and aesthetics of phalloplasty girth techniques.

Silicone implants seem to provide a very smooth, and obviously permanent result. Similar to grafting methods, any surgery that introduces an implanted material will introduce serious post-op risks.
From reports, healing with penuma silicone prosthesis implants is very patient involved and aftercare is also very intense and can last for a few months. Due to the risks and intensive aftercare, silicone implants are not an option for me.
Ventral Phalloplasty
Ventral Phalloplasty is a method I've actually considered. Ventral phalloplasty is penoscrotal web reduction. Penoscrotal web is the flabby, extra skin that connects the testicle sack to the ventral, or underside base of the penis. This surgery doesn’t introduce any implants, or grafts and seems to have a high patient satisfaction rate. Without the use of grafting, or implants the safety factor of phalloplasty increases. Although watching the following ventral phalloplasty video the surgery looks intense and probably has a good deal of uncomfortable healing, but compared to grafting, the ventral penoscrotal web reduction is probably much less patient involved in regards to aftercare.

Penis Lengthening Surgery
Penis lengthening is generally accepted as a safe procedure, and like ventral phalloplasty no implants, or tissues are introduced to the penis. Recent improvements that I have witnessed since my surgery are advancements in scar placement, and lengthening surgery is now performed with traction and in the erect state.
From my surgery, I have a transverse scar, and I think it would make sense to gain suspensory ligament access via the same scar if I ever considered gaining more length. If I was having lengthening for the first time, I would whole heartedly opt for the Surgery LIFE Enhancement method where the surgeon utilizes a testicle skin incision for lengthening, but an additional scar would be necessary in my case.
